To help our patients without dental insurance, we have designed an affordable alternative to maintain your preventative care.
This plan is not dental insurance, and it is exclusive to our office. It will give you the opportunity to visit our office regularly for your preventative care. Any restorative work that is needed will be discounted.
We believe that preventative care is essential for long-term oral health. Our hope is that this membership plan will keep your smile healthy.
Brilliant Smiles Membership Plans are designed to provide affordability and treated access to quality dental care.
With your discount plan, there are no yearly maximums, deductibles, claim forms, pre-existing condition limitations, or waiting periods.
Child Plan (Ages 0-11)
- 2 Cleanings (Prophylaxis) per year
- Oral cancer screening
- All Periodic, Comprehensive exams covered

Adult Plan (Ages 12 and up and absence of periodontal/gum disease)

Perio Plan (Patients with periodontal/gum disease)
- 4 Cleanings (Periodontal maintenance)
- Oral cancer screening
- All Periodontal, Comprehensive Exams

Program Terms and Limitations
Program Terms and LimitationsThis program is a discount plan, not a dental insurance plan. It cannot be used in conjunction with any other insurance or discount plan.
Brilliant Smiles membership plan can only be used at Brilliant Smiles Family Dentistry located in Cadillac, Michigan. If you are referred to a specialist, discounts will not apply.
This plan is non-transferable; family members cannot be substituted for another family member.
Dues are Non-Refundable. No refunds will be given if the patient does not use their dental plan.
This plan and its rates are subject to change yearly.
Exams and preventative care must occur within the year of enrollment and cannot be carried over into the following year.
No-Show or late cancellation fees ($50) will apply to any failed appointment or cancellation without 24 hours’ notice.
Your effective date is the date you pay in full, and your renewal date is the same date every year. You will not receive a card; your membership effective date will be on file with our office.
The plan cannot be used for services related to a Workman’s compensation or Automobile coverage treatment.
All payments are due at the time of service via cash, check, or credit card to receive the discount. Any services that are not paid for at the time of service will be billed at regular free.
